[S390] add z9-ec/z10 instruction to kernel disassembler Add the instruction of the z9-ec and z10 machines to the kernel disassembler. Add the missing "ptff" instruction of z9-109 and the missing "sqd" of g5. Remove useless comments with instruction examples from format table.
