[Bluetooth] Cleanup of the HCI UART driver This patch contains the big cleanup of the HCI UART driver. The uneeded header files are removed and their structure declarations are moved into the protocol implementations. Signed-off-by: Marcel Holtmann <marcel@holtmann.org>
